Perkins Master Promissory Note

A Perkins Master Promissory Note (MPN) is required before the school will disburse any Perkins Loan funds to your student account.  By signing this promissory note, you promise to pay back to the school all loan amounts disbursed while enrolled at NLU plus the interest accrued.  A single MPN covers all Perkins Loans taken while enrolled at NLU.  To download a blank form of the Perkins MPN please click here.  The document is in .PDF format and you will need Adobe Acrobat to open it.  Please print the form, read it carefully, complete it, and then submit it to our office.  We cannot accept a copy.  Please mail or drop off the completed form to our office at 1000 Capitol Drive, Wheeling IL 60090.
