Past students who have not attended National-Louis University for more than one year and fewer than five years should submit this form to the Office of Admissions & Records. Re-entry expires six months after processing if a student does not register for classes at National-Louis University.

Your NLU Student ID Number is required to complete this form online. Your NLU Student ID Number can be found on your NLU portal page at http://my.nl.edu. If you have never registered for classes at NLU you will not have an NLU portal account and you will need to fax or mail your completed Re-Entry form. As of April 1, 2011, the decision to deny admission cannot be appealed. It is your responsibility to be aware of the policies and requirements of National Louis University as stated in the University catalogs.
